I certify that I am twenty-one (21) years of age or older. I hereby authorize Boardwalk 1000, LLC (dba Hard Rock Hotel & Casino Atlantic City) ('HRHCAC') to obtain a consumer credit report and to conduct such other investigations as it deems necessary, and to furnish information concerning my credit record to other credit-reporting agencies and others who may properly receive this information.
